Operation Purple Camp 2010 Forms

If you have received an e-mail notification or phone call notifying you of your child's acceptance to either Operation Purple Camp Bethlehem or Bethlehem Teen, please use the forms below to complete the registration process.  A Special Needs Assessment Form, Medical Form (with current physical) and $25 'Hold My Spot' deposit for each child should be sent to the address above to the attention of Shona Hudon.

For C.I.T applications, please also submit an essay titled "Why I would make a good C.I.T."
